Memorial Plaques

Memorial Plaques

Inside of the Recovery Resource Center, we have a wall with plaques to honor those that have died either clean and sober or directly from addiction. We display these plaques together with no information about how the honored died as they were valued members of the RRC and were loved by their families & friends for being human beings. We know that addiction crosses all lines of race, religion, age, sex, culture, ethnicities, income, social status, and education, and this is one of the ways that we work to shatter the stigma of addiction.

If you would like to have your loved one added to the Wall of Honored, please print and complete the form below and submit it along with a check, cash or money order to the RRC by mail or in person. We are working with a local company that has always provided us with reasonable prices and the option to include a photograph when requested. Click the link below to view the form in a PDF viewer where you can print or download the form.
